We present a 74-year-old male with persisting acute chest pain, inferior wall ST elevation on ECG and polymorphic ventri- cular tachycardia, who had simultaneous coronary spasm of both the right coronary artery and the left anterior descending artery in the absence of significant coronary artery stenosis. We also discuss the relevant literature.
